TERMS AND CONDITIONS
The lab results relate only to the
samples collected through our walk-in or home collection
facility.
For the tests performed on specimens
received from other locations, it is presumed that the specimen
belongs to the patient named or identified, such verification
having been carried out at the point of generation of said
specimen.
Special tests may be outsourced, and
in such cases, the overall quality of the external labs shall be
verified by looking into their accreditations. But it is
impossible to verify each instance; therefore, in case of
discrepancies within the outsourced tests, external lab shall be
held responsible.
There may be instances where the
requested test cannot be performed due to reasons like
- Specimen quantity insufficient
- Specimen quality unacceptable
(homolysed/clotted/lipemic etc.)
- Incorrect specimen type
- The test may be cancelled on request of the patient
- Specimen received without an approved requisition
- Requisition received without a specimen
- The requisition or specimen label lacks two patient
identifiers (name, age, sex, phone number)
- Requisition or specimen label information is illegible
- Requisition and specimen label information is not
identical
- Requisition or specimen mislabelled (patient
identifiers inaccurate)
- Incorrect specimen container/tube is used
- The date of collection is not recorded
- The time of collection is not recorded
- Specimen clotted when an anti-coagulant is added
- The specimen is too old for testing
- The specimen container is leaking
- Specimen contamination, dilution, or other interfering
substances affect specimen integrity
- Inappropriate specimen
- Duplicate test request
